Checkups for children and teens
Child and Teen Checkups is a program for children and young people under the age of 21 who are on Medical Assistance. We are here to help you:
- Find a medical or specialty clinic
- Find a dentist
- Schedule appointments
- Arrange transportation and interpreter services for your appointments
- Connect families to community resources
Some families will receive this help through their clinics. We will assist all families to find out where they can call for help.

We have multilingual staff and interpreters available to assist.
Make a referral
Agencies and parents or guardians may refer a client or child for assistance by filling out this secure referral form.
Note for subcontracted agencies: referral forms should be submitted by the sub-contracted agency within 24 hours.
Appointments and checkup milestones
We help make medical and dental appointments and arrange transportation and interpreter services to clinics that provide regular health checkups for children and young people under the age of 21.
Appointments can include:
- A complete physical exam
- Hearing and vision checks
- Lab tests and immunizations
- Developmental, growth and mental health screenings
- Regular dental appointments
- Information about physical and mental health, healthy habits, nutrition and more
- Health checkups can be used meet Head Start, WIC, school, sports or day care enrolment
It is important for children and young people to get health checkups at these milestones:
- Between birth and 1 month
- At months 2, 4, 6, 9, 12, 15, 18, 24, 30, and 36 months
- At least once a year after age 3 through age 20
