Home visits for families
Hennepin County works with health care providers and local community agencies to offer family home visiting programs for eligible residents at no cost to families.
These programs send public health nurses, and other professionals to visit families in their homes or other community places to:
- Help families adjust to a new baby
- Provide postpartum care
- Assist pregnant people and families in getting resources
- Help families bond
- Make sure babies and kids develop and grow up healthy and safe
- Prepare kids for school
Participation and referrals
We accept referrals from families and a variety of health professionals, programs, and organizations. These include nurses, physicians, social services providers, WIC, and health plans.

Family Connects
Postpartum nurse visits open to all people who give birth at Hennepin Healthcare’s Birth Center who also live in Hennepin County.
Postpartum and newborn home visiting
Skilled nursing assessments of newborn and postpartum client.
Pregnancy support
Assessment, teaching, and support.
Long-term home visiting
Professional home visiting support for families with children up to age five.
