Home visits for families

Hennepin County works with health care providers and local community agencies to offer family home visiting programs for eligible residents at no cost to families.

These programs send public health nurses, and other professionals to visit families in their homes or other community places to:

  • Help families adjust to a new baby
  • Provide postpartum care
  • Assist pregnant people and families in getting resources
  • Help families bond
  • Make sure babies and kids develop and grow up healthy and safe
  • Prepare kids for school

Participation and referrals
