Get connected with services to keep your family safe, stable, and healthy.

These programs are voluntary.

Children under age 10

The Parent Support Outreach Program (PSOP) is a short-term, voluntary program. You, as the parent, decide what you want to work on.

We can connect you with help to meet your family's urgent needs and support you in reaching your goals.

Connections to resources

We can help you find support and assistance for:

  • Mental health and crisis counseling
  • Resources to stay clean and sober

Basic needs

  • Food and clothing
  • Help navigating housing resources
  • Safety from violence at home
  • Academic success

  • Help applying for financial resources
  • Job training and search
  • Money management

How the program works 

You will partner with a social worker to address your family’s needs. Your PSOP social worker will offer you support and resources to meet your goals.

Eligible candidates

  • Live in Hennepin County
  • Be pregnant or have at least one child 10 years old or younger
  • Need help with issues that make it hard to be a parent, such as financial problems, substance use, mental health, domestic violence, housing instability or homelessness

Youth ages 11 to 17

The Youth Support Outreach Program (YSOP) connects youth ages 11 to 17 with support to address concerns that put them at risk, like substance use and mental health concerns, or sexual exploitation.

Social workers focused on youth well-being collaborate with parents or caregivers and youth to create a plan to work towards safety and healing.

YSOP is a voluntary program, separate from the child protection or juvenile justice systems.

Plans could include:

Health and well-being

  • Mental health and crisis counseling
  • Services and support for substance use
  • Physical and sexual health services
  • Support for trauma survivors

Family stability

  • Crisis response and stabilization
  • Specialized services, such as Functional Family Therapy or Multisystemic Therapy
  • Collaboration with existing supports, like school and health providers
  • Connections to community resources and other services

How the program works

There are no fees for case management services provided by YSOP social workers. Other services may be covered by your health insurance. Parents/caregivers will sign releases of information to coordinate services.

Youth sexual exploitation

Sexual exploitation of youth is a form of violence against young people. It involves the exchange of sexual activity for something of value: money, drugs, food, or shelter.

Through our partners in the community, we can help you access a variety of services. Sexually exploited youth are treated like victims, not criminals.

Family resource centers

Family resource centers are welcoming spaces where parents can get resources at low or no cost. Families get support from community-based providers to keep their family safe and healthy.

Services and resources are chosen by members of the local community and vary at each location. All programming focuses on supporting families in five key areas:

  • Basic needs
  • Social support
  • Parental resilience
  • Understanding child development
  • Children’s social and emotional needs

How it works

Family Resource Centers are open to the public, and accessing resources is voluntary. Parents choose the support that’s right for them and their family.

Staff members support parents to reach their own goals with service navigation, check-ins, and referrals.

Support is available in person as a one-time drop in or during regular meetings.

Staff are members of the local community who share the backgrounds and lived experiences of families in the area.
