Help for unsheltered homelessness


If you are sleeping outside, in a tent, vehicle, or other places not meant for human habitation, you can get connected to supportive services. This includes connections to shelter and housing through the Streets to Housing program.

Services

The Streets to Housing team serves single adults, youth, and families in Hennepin County, with trauma-informed, housing-focused services that reduce barriers to housing and identify safe, appropriate alternatives to living outside, including emergency shelter and permanent housing. Streets to Housing can assist with needs including:

  • Navigating the homeless response system
  • Getting vital documentation and connecting to benefits as you are eligible
  • Filling out applications for housing, including Coordinated Entry System (CES) assessments
  • Making a referral to case managers, including housing-focused case managers

Eligibility

To avoid duplication of services, Streets to Housing won’t enroll someone who is already working with another street outreach team in Hennepin County. Streets to Housing serves residents who meet all of the following criteria:

  • Experiencing unsheltered homelessness, which means you are sleeping in a place not meant for human habitation such as cars or abandoned buildings
  • 18 years old or older

This team specializes in serving people with no safe place to stay. The team doesn't work with people who are staying with friends or family, who lack a lease, or whose housing is unstable.
