Community support program
Adult residents of Hennepin County with a diagnosis of a severe and persistent mental illness may join Community support programs. Staff help people:
- Find and maintain competitive employment.
- Handle basic activities of daily living.
- Participate in social activities.
- Set goals and plans.
- Get and maintain appropriate living arrangements.
Join a community support program
- No referral needed. Go on a day the drop-in location is open.
- Bring a copy of a diagnostic assessment stating your diagnosis. Or have your login information so you can access it online.
