Changes to Medical Assistance
Federal law has changed who can get and keep Medical Assistance benefits. Additional changes are coming in 2027. The state has already reached out to those who are affected. Most people's coverage will not change.
Services are available to seniors and people of all ages with a disability. We can help you understand your options and connect you to services that meet your needs.

Services are available to eligible seniors and people of all ages with a disability.
Getting help to stay in your home
These services help people stay in their home. Without these services they might be at risk of having to leave their home.
Services offer a way of life other than in an institution. They help people live as independently as possible.
Examples of services

- Home-delivered meals
- Living skills training
- Services such as cleaning and meal preparation
- Respite care
Getting supportive services in the community
These services help people feel connected to the communities they live in.
Services are an alternative to institutionalization, helping people feel integrated in their community.

Consumer Directed Community Supports (CDCS) overview
This training provides a comprehensive overview of the Consumer Directed Community Supports (CDCS) program and how to get started.

Consumer Directed Community Supports (CDCS) question and answer sessions
Current or future CDCS participants can stop in any time during these one-hour sessions to ask questions about Hennepin County’s CDCS process or writing their CDCS plan. We can't answer specific questions about approved or denied items.

Family support grant
The Family support grant program provides cash grants to families of children with certified disabilities who are not on a waiver or receiving Community first services and supports. The program provides access to family-centered services and supports specific to the child’s disability.
This session will explain the basics of the Family support grant program and how to complete the application to ensure program compliance. Case managers and families who want additional information to apply for their eligible child are welcome to attend.

Transition Extravaganza
Learn about services available for transition age people with disabilities (ages 16-23). This event will have a resource fair and presentations about transitioning to adulthood.
Families, caregivers, case managers and school staff are welcome to attend. Refreshments will be provided.
